第二外語考試申請書(音樂學組)Second foreign language examination application form (Musicology Group)：google表單填寫完成後，資料會自動寄至同學於表單中所填寫的電子信箱，請印出並親簽（若兩頁以上請以雙面印刷），送至師大音樂系辦（教務辦公室），謝謝！After completing this google form, the information will be automatically sent to the email address filled in by the students, please prinit t out and make sure it’s signed personally (if more than two pages, please print on both sides). After you confirm that the advisor and the convener have signed, please send it to the Music Department Office (Departmental Affairs Office) of Normal University, thank you!
